Dependent Verification Review

All benefit eligible employees should have received a letter from the Human Resources Department regarding the mandatory Dependent Verification Review and HR Check-Up. Below, you may download a copy of the letter that was sent out.  

The verification review will take place August 25 through September 5, 2014 at the Fremont campus. However, American Fidelity Assurance (AFA) counselors will also be at the Newark Campus on August 28 and August 29, 2014 (all day). Please call (800) 365-8306 ext. 0 and make an appointment to meet with an AFA counselor at a location that is convenient for you. Each employee who is eligible to be on a District medical, dental or vision plans will be required to meet with an AFA benefit counselor to confirm eligibility of each dependent by providing them with supporting documentation (PDF).

You must meet with an AFA benefit counselor and complete your verification review by the end of the review period. Failure to complete this review process will result in the termination of dependent coverage effective October 1, 2014. Be aware that there will be no prosecution or discipline for fraudulent dependents by the district whether their coverage was purposeful or accidental.

In addition to dependent verification, we have asked AFA to provide you with information on the new pre-tax transportation benefit (also known as Section 132) (PDF) that will be available to you starting September 1, 2014. Furthermore, some of you have expressed interest in voluntary offerings for short and long term disability benefits and additional life insurance. If you would like to learn more about those benefits you have the option to use this meeting to have those benefits explained to you. You are not obligated to receive information or purchase any of the available voluntary benefits, but we are required to provide information about the new commuter benefit.

HR Check-Up

Following your appointment with the AFA counselor you have the option of meeting with one of us in HR to review and update your personal and emergency contact information, discuss any life events you may have experienced that may have caused you to change your beneficiary for your STRS/PERS, district sponsored or voluntary life insurance.
