Faculty: Please use the Course Assessment in a Box form to complete your Course Assessment report. Add any additional files to the bottom of the form. Send completed reports to your dean at the end of each semester once you have collected and analyzed your data (or during Get It Done Day at the beginning of the following semester). Please save filenames in this format: 2013spring-sloa-engl101a. (Please do not include student names in the data.)
