10/17/2017 Fremont campus, building 5, elevator next to bathrooms The inside of the elevator on the walls and floor there is a massive amount of mold and deterioration. The mold and mildew is deeply imbedded in the carpet and there are large stains everywhere. It is very hard to breathe because of the strong smell. It also has not been inspected since 2015. On 10/18/2017, inspection was completed. Carpet and paneling was removed and no signs of mold or mildew. Elevator was thoroughly cleaned. The State's Department of Industrial Relations (OSHA) last inspected the elevator on 9/1/2016. The most recent copy was removed, however, a new copy of the last inspection was placed in the elevator.
09/19/2017 Newark Center, NP10 Ramps to NP 10 is cracking and falling a part Created a ticket with facilities to make the necessary repairs to the flooring.
07/06/2017 Fremont, Building 6 room 6106 Parts of the carpet is unraveling and coming off the floor causing a potential trip hazard. Created a ticket with facilities to make the necessary repairs to the flooring.
06/28/2017 Fremont, FP 32 Gripping on the ramps to FP 32 has worn off. Contacted vendor to repair the gripping to the mailroom ramp. Waiting on ETA.
04/12/2017 Fremont, Mailroom Gripping on the ramps to the mailroom has worn off. When wet, this is slippery. Contacted vendor to repair the gripping to the mailroom ramps.

On 04/12/2017, mats were placed on top of the worn off areas and warning signs were placed upon entrance and exits of the ramp. Work on repaving the ramps were completed on 04/22/2017
04/08/2017 Fremont, Building 7 In building 7, certain offices have reached temperatures up to 81 degrees. Problems with the temperatures could be related to lack of HVAC maintenance.

The complaint was valid and has been corrected:

  • Permanent repairs were made to the HVAC on approximately 03/17/17.
  • Restoring the HVAC system in the Placement Center will reduce heat/cooling variation.
  • Communication between Director Guillen and Placement Center staff will assist in reaching the desired indoor air quality in this area.
  • Independent temperature monitor will yield need for further adjustments.
  • End-user education on not blocking floor vents or placing heat-producing equipment (computer monitor, etc...) near wall thermostats will help reduce false temperature readings
  • CO2 monitors will be examined for calibration needs.
  • Twice daily, HVAC technicians will check the Assessment Center to address temperature needs
03/29/2017 Fremont, Smith Center Alleged Dust and hazardous mold in the Smith Center A 3rd party industrial hygienist from Keenan & Associates conducted an environmental test on 04/21/2017 & 05/01/2017. Results came in on 05/08/2017 and found NO HAZARDOUS MOLD in the Smith Center (see PDF report).
December 16, 2016 Corner of Pine and Mission; and Front of Campus Police We were walking we noticed these wood fences with totally unfinished wood (not attractive at all and dangerous). Not sure what they are for, but worry that it is a BIG liability for the district if someone gets hurt. We noticed one at the corner of Pine and Mission and one in front of the Campus Police building. Facilities and Campus Police has been contacted on December 16, 2016th to look into this concern
October 28, 2016 NP-6, Newark campus I think that there's a wasp nest near NP-6 (sometimes wasps are inside the classroom) and I have a student who is allergic to wasps. She's pretty concerned for her safety so I emailed Ohlone's Workplace Safety Committee.

Update: December 1, 2016: Clark Pest Control did come out and look at FP# 6 in search of a possible wasp nest near or on that portable. The area was checked out and there was no actual nest to be found. A point of entry was sealed on the portable and treatment was applied. The surrounding area was also surveyed for possible evidence of wasp activity and no such activity was found.

October 28, 2016 - Facilities updated me and said they contacted Clark Pest Control yesterday afternoon and asked that they come out to address the wasp issue at NP#6 as soon as possible.

October 27, 2016 FP-15, Fremont campus broken light in classroom FP-15 (Fremont Portable) on the main Ohlone Campus. By the door there are 3 light switches. Turning on the middle light switch results in an intermittently flickering light that is so distracting the light has to be turned off, resulting in a dim classroom Facilities has been contacted on October 28th to look into this issue and concern
October 22, 2015 Parking Lot B, Fremont campus The asphalt in parking lot B is so rough and broken that I've almost turned my ankle a few times and am worried about falling.
September 15, 2015 NP-6, Newark campus I received a call today from an instructor in NP 6. Her class is in the afternoon Tues and Thurs, from 1345-1520 hours. She claims the room is hot and the air conditioning is not working. Can you please have someone check out the class room today.

September 15, 2015 Update - We were able to check on NP-6 and the classroom was full with students, and the temperature was a mild 73 degrees. A quick survey was taken by the professor asking about the room temperature and it was an overwhelming "it's fine". From the looks of the thermostat, the device is functioning fine with a temperature set at 72 degrees until 10pm, where it's set back to something warmer.

September 15, 2015 - Facilities is having an HVAC tech look into this problem.

September 14, 2015 Pond area, Fremont campus Almost everyday, someone drives up to the pond area and park his/her silver van there for an extended period of time. I'm concerned for the safety of the students and staff who need to walk in that area. If it is someone from the cafeteria, please let him/her know that there is a loading zone next to the cafeteria where he or she can park safely.

September 15, 2015 Update - The District has identified who this van belongs to and will discuss this issue further at the next Safety Committee Meeting in September.

Newark classrooms in general

The floor plugs in the Newark classrooms used as the IT power source, have been left in a semi-opened position several times. This is a safety issue. There has been three (3) incidents where someone has tripped and sustained injuries. March 17, 2015 - The District is looking into this.

Walkway from Building 7 past Buidling 9, Fremont campus There are lights that are out as well as the overall darkness of the path and stairs. The lights have been out for a long time and other employees have submitted other requests to have them addressed.

March 30, 2015 Update - The District is continuing to enhance and add more lighting. Please see the picture showing new pathway lighting provided at the North side of Building 9. These LED fixtures are operated by photo-sensor and should provide adequate lighting for the adjacent pathways. They were operational starting 3/25. We will be painting the exposed conduit.

February 19, 2015 - The District is looking into this.

February 3, 2015 From Gym to Staff Parking Lot A, Fremont campus At the last Ohlone basketball game on February 20th, I was going from the gym at 6:30 pm to the staff parking lot A. The pathway was very poorly lit and several of the lights were out. Some of the guests from Chabot seemed confused about the best way to get down to their cars beyond Hyman Hall and Building 19. I was able to help those who left at the same time I did. My suggestion is not only upgrade the lighting in this area, but provide guides after evening events which end after dark to help the attendees get back to their cars. Obviously, basketball is over, but there will be other night time events in Bldgs 9 and 7 where this path will be needed after dark. March 4, 2015 - The District is looking into this.
January 29, 2015 Newark campus: Staff parking lot area The concern is that students/people drive their cars extremely fast near the staff parking space in Newark. There are no four way stop signs and no road bumps to help the cars slow down; we are wondering if a four way stop sign can be placed. Also, we suggest that a pedestrian walk area be painted on the streets so that staff members can walk from Newark campus to the Staff Parking Space. Right now, staff members who park in the staff parking lot in Newark dodge the flying cars and run across the street so that they can get to the Newark Campus. February 6, 2015 - Facilities and Campus Police will be installing stop signs and a crosswalk.

There are some light fixtures out.

New Fremont portables There are uneven pavements. There are extreme dips that are not marked in any way, and are kind of easy to miss. Concerned about students in wheelchairs trying to navigate around people and accidentally going into one of those dips. There are big planter boxes that are spread out all over, kind of getting in the way and creating puddles of water that people will try to avoid.

January 30, 2015 Update - The uneven pavements / dips have been filled with river rocks.

January 28, 2015 - We are looking into this and discussing possible remedies.

December 4, 2014 North Campus Parking Lot, Fremont Campus The north end of the Fremont Campus has no lights around 6:00pm. December 4, 2014 - The lights are in the process of being repaired. They will be turned on 24 hours a day until the controls are repaired.
November 25, 2014 Room 4202, Fremont campus Please be aware that some sort of piping has been installed on the floor in Room 4202. These present a clear tripping hazard for students.

December 4, 2014 Update - The piping cannot be removed as it needs to stay installed as is. A signage has been posted noting construction is in progress.

November 26, 2014 - Facilities is looking into this.

November 16, 2014 Parking Lot P, Fremont campus A large hole on the ground that's big enough that it could be a tripping hazard.

December 1, 2014 Update - The large hole on the ground has been fixed/filled.

November 3, 2014 Building 1, Fremont campus This is to report a foul smell all over the 1st floor. Making some people nauseous.

November 4, 2014 Update - Custodians are cleaning the carpets this morning. So the blowers will be on to dry the area. But the smell should be better than yesterday.

November 3, 2014 - There were several offices on the first floor that had some water damage from the rain. The only way to dry the offices is by using fans. We are hoping that it won't take long for these areas to dry up quickly.

October 16, 2014 Rooms 6202 and 6203, Building 6, second floor, Fremont campus

In 6202, flashing fire alarms were delayed in working…did not go off right away.

In 6203, there are two light flashing fire alarms are not working.

October 15, 2014 - Are there still Building Monitors? I noticed that the list under "Emergency, Evacuation, and Safety Procedures" is dated 2008. November 4, 2014 - Unfortunately, the list is outdated and it will be reviewed as soon as possible. In the meantime, please visit the Safety Committee website to view the names of the Safety Committee members.
October 15, 2014 Evacuation instructions on the Campus Police/Safety and Security website Are the evacuation instructions current and are they only for the Fremont campus?

October 17, 2014 - We have updated the new evacuation routes and gathering areas are updated in several locations of the website. We will update it on the Campus Police/Safety and Security as well.

Building 1, 2nd Floor, north stair case landing, Fremont campus. Near the stair case door next to the HR office.

Plastic grey strip is ripped in the center. A foot can easily catch on the large tear and could cause someone to fall forward.

October 2, 2014 - The old plastic grey strip has been taken out and we are working on getting a new one installed.
September 19, 2014 Newark campus

Floor outlets at Newark are still a huge workplace hazard.

I have complained about this a few times before, and I am aware that the outlets have been improved and now sit flush to the ground when closed. However, they are often left unclosed and present a major tripping hazard when open. I've tripped over them many times myself (sometimes bruising myself in the process) and just yesterday a student tripped over one in the Newark library and practically fell onto my lap. Anyone who teaches at Newark sees students and faculty tripping over these outlets on a daily or weekly basis.

Today Newark hosted a huge consortium and I was horrified to see the Principal of Newark Adult School trip over one of these outlets. I jumped up to help her close the outlet because closing these outlets is not intuitive. In fact, some of the outlets are actually impossible for anyone of normal strength to close and open. I cannot open the one in NC2318 despite applying considerable pressure.

September 10, 2014

The top of the stairs leading up to the common area outside Building 1, Fremont campus.

There are roughly 5 or 6 shotgun shell casings in the grass. The concern is "How did they get there?" They are empty, but do not appear to have been fired. Regardless, these items should not be on campus.

September 10, 2014 - Some concern has been expressed by faculty, staff, and students due to expended rifle and pistol cartridges and expended shotgun shells being found on the upper campus on the Fremont campus. A police training exercise for "Active Shooter Response" was conducted by the Fremont Police Department on August 11th and 15th. The training involved participants firing "training blanks" from rifles, pistols, and shotguns. The Fremont Police Department swept the areas to pick expended brass and shotgun shells. Unfortunately, they did not pick up all of the expended brass and shotgun shells as some have been found by faculty, staff, and students.

Please do not be alarmed if you locate any expended brass or shotgun shells. Contact Campus Police and an officer will respond to collect the brass or shells. There have been no incidents of any persons discharging any type of weapon on or about the Fremont or Newark campus at any time.

August 26, 2014 Parking Lot U, Fremont campus Two of the lampposts in Lot U are not working - they're on the west side of the parking lot, and so the road that goes by Building 24 and Lot V is especially dark there (people walk along that road to get to Lot V). Can we replace the bulbs or fix the lampposts? August 28, 2014 - We are making a valiant attempt to get ALL the outdoor lights on both campuses working.
July 25, 2014 Parking Lot V, Fremont campus Drive way near building 24 needs to be swept daily due to construction debris. Also, the hedges running along the drive way bordering Lot V & Lot U need to be trimmed back at least 2 feet. The hedge is now over the asphalt taking up vehicle driving space and making it a challenge when a pedestrian walk along the drive way at the same time a vehicle is passing. A pedestrian comes really close to the passing vehicle and at times needs to completing move off the road when the driver does not wait for the pedestrian to pass through. It becomes awkward because the road is slanted and mixed with pebbles. August 27, 2014 - On behalf of the district's Workplace Safety Committee, the concern expressed has been corrected.
