INSTRUCTIONS ON COMPLETING THE HEALTH QUESTIONNAIRE

PRIOR TO ARRIVING ON CAMPUS

All employees must complete a Health Screening Questionnaire as part of Ohlone’s access procedures. The Health Questionnaire could be accessed and completed prior to you coming on campus. There are two ways to do this:

  1. Use your smart phone to hover over the QR code. You will be taken to a website where you will be able to access the questionnaire. Complete the questionnaire. Once completed, save a screenshot. You will need to show this to the attendant once your arrive on the Fremont/Newark campus.

  2. You can also complete the questionnaire on your phone or computer. Save a screenshot and show upon arrival.

COMPLETING THE HEALTH QUESTIONNAIRE UPON ARRIVAL TO CAMPUS

A sign with the QR code will be displayed at the checkpoint entrance. (see picture below – the QR code sign is to your left).

   

Use your smart phone to hover over the QR code. You will be taken to a website where you will be able to access the questionnaire. Complete the questionnaire. Show the results to the attendant.

STEP BY STEP INSTRUCTIONS FOR USING THE QR CODE

STEP 1:  USE YOUR PHONE CAMERA’S BUILT IN QR CODE READER.

MOST SMART PHONES HAVE A BUILT IN QR READER OR DOWNLOAD A FREE QR CODE APP. 

STEP 2:  TURN ON YOUR CAMERA AND HOVER OVER THE QR CODE AS IF YOU WERE TAKING A PICTURE.

YOU WILL SEE A SCREEN APPEAR AT THE TOP OF YOUR PHONE. SHOULD LOOK LIKE THIS:

STEP 3:  CLICK ON THE WEBSITE QR CODE AND YOU WILL BE TAKEN TO OHLONE’S HEALTH SCREENING QUESTIONNAIRE

    STEP 4:  FILL OUT THE FORM

    Page 1:

    1. NAME
    2. LAST NAME
    3. PHONE NUMBER
    4. ARE YOU AN EMPLOYEE  (press yes – if not press NEXT)

    PRESS NEXT FOR PAGE 2

    Page 2:

    1. ANSWER 3 HEALTH RELATED QUESTIONS
    2. SUBMIT

    STEP 5:  SHOW THE ATTENDANT YOUR PHONE WITH THE CHECK MARK FOR VERIFICATION

    You will see a check mark on your phone. Show the attendant your phone with the check mark for verification. The attendant will need to see your screen to verify that you answered NO to the questions or if not, verify the information you confirmed as YES.
