INSTRUCTIONS ON COMPLETING THE HEALTH QUESTIONNAIRE FOR STUDENTS AND VISITORS

PRIOR TO ARRIVING ON CAMPUS

All students and visitors must complete a Health Screening Questionnaire as part of Ohlone College's access procedures. The Health Questionnaire can be accessed and completed prior to you coming on campus but it must be done same-day. There are two ways to do this:

  1. Use your smart phone to hover over the QR code. You will be taken to a website where you will be able to access the questionnaire. Complete the questionnaire. Once completed, you will see another QR code. Save a screenshot. You will need to scan this at the symptom screening station at the Fremont/Newark campus.
  2. You can also complete the questionnaire on your computer. Save a screenshot and show upon arrival as long as it is completed same-day.

COMPLETING THE HEALTH QUESTIONNAIRE UPON ARRIVAL TO CAMPUS

Fremont Campus Symptom Screening Stations (see map here):

  • Building 3, Level 4 (Student Study Center)
    • Monday to Friday, 9:00 a.m. to 7:00 p.m., and Saturday, 10:00 a.m. to 4:00 p.m.
  • Building 7, Lobby, and Smith Center, Lobby
    • Monday to Friday, 7:00 a.m. to 8:00 p.m.
  • Hyman Hall, First Floor, Lobby
    • Monday to Friday, 7:00 a.m. to 10:00 a.m. and 3:30 p.m. to 8:00 p.m.
  • Building 19 (for staff only)
    • Monday to Friday, 7:00 a.m. to 10:00 a.m.

Newark Center Symptom Screening Stations (see map here):

  • Courtyard Entrance
    • Monday to Friday, 7:00 a.m. to 8:00 p.m.
  • Floor 1, Room NC 1124 (Student Study Center)
    • Monday to Friday, 9:00 a.m. to 7:00 p.m.

A sign with the QR code will be displayed at the checkpoint entrance. The QR code signs are to your left.

Health Screening Station at Fremont Campus Building 7 (Indoor)

Use your smart phone to hover over the QR code. You will be taken to a website where you will be able to access the questionnaire. Complete the questionnaire. See below for details.

Scan the results at the Turing Machine. Follow the instructions to have your temperature taken. You will be given a colored wrist band upon completing the screening process.

STEP BY STEP INSTRUCTIONS FOR USING THE QR CODE

STEP 1:  USE YOUR PHONE CAMERA’S BUILT IN QR CODE READER.

Most smart phones have a built-in QR code reader; download a free QR code app if needed. 

STEP 2:  TURN ON YOUR CAMERA AND HOVER OVER THE QR CODE AS IF YOU WERE TAKING A PICTURE.

You will see a screen appear at the top of your phone. It should look like this:

STEP 3:  CLICK ON THE WEBSITE QR CODE AND YOU WILL BE TAKEN TO OHLONE COLLEGE’S HEALTH SCREENING QUESTIONNAIRE

Be directed to the Health Screening Questionnaire page.

    STEP 4:  FILL OUT THE FORM

    Page 1 - Personal Information

    1. First name
    2. Last name
    3. Phone number
    4. Employee / Non-employee

    Page 2 - Health Conditions

    1. 3 health related questions
    2. Submit

    STEP 5:  SCAN THE QR CODE PROVIDED AFTER SUBMISSION TO MACHINE AT SYMPTOM SCREENING STATION

    You will see a QR code on your phone after submission. Scan that code to the machine at the screening station for verification. After scanning you will need to align you face with the camera and your temperature will be taken. After verification the health screener will provide a colored wrist band.
